Care planning is essential to the delivery of high quality health and social care provision, which meets the needs of individuals. The aim of this module is to enable you to understand the care planning process and its importance for health and social care.

 

During the module you will consider the importance of a needs assessment, planning, implementation and reviewing of care plans. You will do this through the consideration of important theory, including examining the nursing and social care models.

 

You will also be given the opportunity to explore Person Centred approach / Anti-discriminatory Practice and cultural understanding. National and Local Policy relating to Planning Care Delivery will also be examined. The learning will take place through a combination of lectures, workshops and tutorials, with the aim of helping you understand how theory and practice link together.
